I the parent/guardian give my Student permission to
attend Transformation Camp 2025. I release the camp and/or staff from
any liability and acknowledge that all questions/information are
answered honestly.


Youth Medical History

Must be completed By Parent or legal Guardian 

Does your son/daughter suffer from any medical, physical, emotional, or behavioral conditions which might affect his/her safety while at camp? (e.g., claustrophobia, vertigo, asthma, heart condition, diabetes, epilepsy, etc.)

Is your son or daughter currently undergoing any form of medical Physiological treatment including medication? 

Will you be bringing any prescriptions or non-prescription medication to camp?

This must be turned in at check-in the first day of camp or mailed in with your registration form.
Is your son or daughter allergic to any food, medication, or insect stings?

List any surgeries or Serious injuries in the last 2 years
